After application has been submitted by applicant, a Meridiem DermSpa team member will contact you to schedule a free in-office treatment assessment.
In-office treatment assessment must be completed by February 28th, 2022.
• Must be 18 years or older to enter
• An in-office assessment must be completed no later than February 28th, 2022, to be eligible to win
• Winner must have treatment(s) by March 31st, 2022.
• Winner agrees to document their experience with photos, videos, interviews and posts on various Anne Arundel Dermatology and affiliate offices digital platforms.
• Winner will be required to sign Photo/Video Media Release Form.
(ie. before & after photos, footage during treatment, testimonial videos, etc.)
• Winner will be required to sign Treatment Consent.
• No cash alternative will be offered. FREE treatments are not transferable.
• If selected winner does not respond to either phone or email contacts made by the office to inform him/her of being selected as the winner within 7 days, then another winner shall be chosen, with no further entitlement to the initial winner.
