Patients who are new or existing will need to complete the necessary forms before their visit along with submitting a copy of their current insurance cards and photo ID. Patients will receive a text through our secure, HIPAA-compliant platform Klara, that will include all forms needed. Once the patient receives, they will view, sign and submit all forms using this platform. We encourage paperwork to be completed digitally for a seamless office experience.

Face masks are optional throughout the visit in office. Please arrive 15 minutes prior to your appointment to check-in.

Ensure you have completed your pre-appointment paperwork, through the secure Klara text process, for a seamless check-in experience. If you did not complete the mobile paperwork process, please plan to arrive 15 minutes prior to your appointment time to complete in office. All patients should proceed to the front desk to confirm your arrival and complete the check-in process. Insurance copayment and payment for self-pay services will be collected at time of service.

Once checked in, the clinical team will be notified to call you from the waiting room.

After your visit, our check out team will assist in scheduling all recommended follow up appointments. Please note that ALL aesthetic procedure fees will be collected, in full, at time of service. For insurance visits, estimated deductible and co-insurance will be determined and collected at time of check-out.
