What Is Topical Chemotherapy?

When most people hear "chemotherapy," they think of infusions given in an oncology clinic, a very different experience from what is described here. Topical chemotherapy, as used in dermatology, refers to prescription creams applied directly to the surface of the skin to destroy precancerous or cancerous cells locally. There are no IV lines, no hospital visits, and no systemic side effects associated with traditional cancer chemotherapy.

The most commonly prescribed topical chemotherapy agents in dermatology are 5-fluorouracil (5-FU), available under brand names including Efudex and Carac, and imiquimod (Aldara and Zyclara). Both work by interfering with the growth of abnormal cells at the skin's surface. 5-FU blocks the cellular processes that allow abnormal cells to replicate. Imiquimod works differently, stimulating the immune system to recognize and attack abnormal tissue.

Your dermatologist will prescribe the appropriate medication, concentration, and application schedule based on your specific diagnosis and the area being treated. The treatment course typically spans several weeks and is managed at home, with follow-up visits to monitor your response and confirm resolution.

Who’s a Good Candidate?

Topical chemotherapy is best suited for patients with surface-level abnormal skin cell growth across a defined treatment area.

Your dermatologist may recommend it if you have:

Multiple actinic keratoses

Superficial basal cell carcinoma in an area appropriate for non-surgical treatment

Squamous cell carcinoma in situ (Bowen's disease) confined to the outer skin layers

A treatment area with widespread field damage

A preference or clinical suitability for a non-procedural treatment approach over in-office destruction or surgery

Topical chemotherapy is not appropriate for deeper, invasive, or high-risk skin cancers, or for lesions that require tissue removal and pathologic margin confirmation.

What Results Can I Expect?

For the right candidates, topical chemotherapy is highly effective at eliminating precancerous lesions and superficial skin cancers across a treatment area. One important advantage over spot treatments like cryosurgery is that topical agents can treat both visible lesions and subclinical abnormal cells in the surrounding field.

The treatment process involves a visible inflammatory skin reaction that can be significant. This response is expected and is a sign that the medication is working.

Your provider will prepare you for what this looks like and help you manage any discomfort during the course of treatment. Most patients complete healing within eight to twelve weeks of finishing the application period.

THE DAY OF YOUR PROCEDURE

What to Expect With Topical Chemotherapy?

Unlike in-office procedures, topical chemotherapy is a multi-week treatment managed primarily at home. Here is how the process works from your first consultation through the end of your treatment course.

Consultation and Diagnosis

Your dermatologist examines the area of concern, confirms your diagnosis, and determines whether topical chemotherapy is appropriate. They will select the right medication, most commonly 5-fluorouracil or imiquimod, and establish your application schedule, treatment area, and duration.

Prescription and Instructions

Before you leave, your provider walks you through how to apply the medication, how often, and for how long. You will receive clear guidance on what to expect during treatment, including the skin reaction that is a normal and expected part of the process, and what to watch for that would warrant a call to the office.

At-Home Application

You apply the prescribed cream to the treatment area at home according to your provider's instructions. The frequency and duration vary by medication and diagnosis. 5-FU is typically applied once or twice daily for two to four weeks; imiquimod regimens vary and may extend over several weeks. Wash hands thoroughly after each application.

Inflammatory Response

As treatment progresses, the treated area will become red, inflamed, and may crust, blister, or feel raw. This reaction is expected and indicates the medication is working. Your provider will advise you on how to manage discomfort and protect the skin during this phase.

Follow-Up Monitoring

Your dermatologist will schedule one or more follow-up visits to assess your response to treatment, address any concerns about the skin reaction, and confirm that the treatment is progressing as expected.

End of Treatment and Healing

Once the application course is complete, the inflammatory response gradually subsides and the skin heals. Your provider will evaluate the treated area to confirm resolution and determine whether any additional treatment or ongoing monitoring is needed.

What to Expect During and After Your Treatment Course

Topical chemotherapy does not follow a single-day recovery arc; it unfolds over weeks, with the most visible skin reaction occurring mid-treatment before healing begins. The timeline below reflects the general course of a standard treatment regimen. Your provider will give you a specific schedule based on your diagnosis and the medication prescribed.

Recovery At A Glance

Week 1-2

You begin applying the prescribed cream at home as directed. The skin may look normal at first. Mild redness or tingling in the treated area is common as the medication begins to work.

Weeks 2-4

The inflammatory response intensifies. Treated areas typically become red, swollen, and may blister, crust, or feel raw. This reaction is expected and indicates the medication is targeting abnormal cells.

Weeks 4-6

Application is typically complete. The skin begins to heal. Redness, crusting, and irritation gradually subside as treated areas start to clear. Your provider may schedule a check-in during this phase.

Weeks 6-12

Most patients see full healing by eight to twelve weeks after completing the course. Your provider will evaluate the treated area to confirm resolution and discuss any follow-up care or monitoring needed.22

I had a wonderful experience with Danielle Johnson, PA-C at Virginia Dermatology. Scheduling an appointment through the patient portal was very easy and was able to be seen within a few weeks. I was pleasantly surprised when the office even called to offer me an earlier appointment. When I arrived, check-in was quick and I had hardly any wait time. I only waited a few minutes before Danielle came into the room. Overall great experience. The service they use to book appointments and acquire documents online works fast and smoothly. Front desk was warm and welcoming. I waited about ten minutes which was no big deal. The physician quickly assessed my concerns and made sure I understood her explanation/diagnosis. I was made to feel comfortable by admissions and treated very kindly with respect during the biopsy.. I recently moved to Leland and left a wonderful dermatologist in my hometown. I needed to find a dermatologist in my new hometown and was so afraid I could find nothing to compare. We are thrilled to have found VA dermatology. After moving to virginia from vegas, we tried parasier dermatology, but they were awful and did not provide the same quality as our previous vegas dermatologist. The chemotherapy used in oncology is systemic: it is administered intravenously or orally and travels through the entire body to target cancer cells wherever they may be. Topical chemotherapy in dermatology is applied directly to the surface of the skin and works only in the area where it is applied. It does not enter the bloodstream in meaningful amounts and is not associated with the systemic side effects that most people associate with traditional chemotherapy.

The two most commonly prescribed topical chemotherapy agents in dermatology are 5-fluorouracil, available under brand names including Efudex and Carac, and imiquimod, available as Aldara or Zyclara. Both are prescription medications. 5-FU works by blocking the ability of abnormal cells to replicate. Imiquimod works differently. It activates the immune system to identify and eliminate abnormal cells. Your dermatologist will select the appropriate medication and formulation based on your specific diagnosis.

Topical chemotherapy is most commonly used to treat actinic keratoses, or precancerous, rough, sun-damaged patches that can progress to squamous cell carcinoma if left untreated. It is also used for superficial basal cell carcinoma and squamous cell carcinoma in situ in areas appropriate for non-surgical treatment. It is particularly well-suited for treating field damage, broad areas of sun-injured skin with multiple visible and subclinical lesions.

A significant inflammatory skin reaction is a normal and expected part of topical chemotherapy. As treatment progresses, the treated area typically becomes red, swollen, and may crust, blister, or feel raw. This reaction can look alarming if you are not prepared for it, which is why your provider will explain exactly what to expect before you begin. The reaction indicates that the medication is working. If you experience severe pain, signs of infection, or a reaction outside the treatment area, contact your provider.

Most patients can maintain their normal routine during treatment, though you may want to avoid situations where the treated area will be prominently visible or exposed to the sun. Sun protection over the treated area is important throughout the course. Your provider will advise you on any specific activity restrictions based on the location being treated.

The duration varies by medication and diagnosis. A standard course of 5-fluorouracil is typically two to four weeks of daily or twice-daily application. Imiquimod regimens may span several weeks with application on alternating days. Full healing after completing the treatment course generally takes an additional four to eight weeks. Your provider will give you a specific schedule at your consultation.

The frequency of follow-up visits will depend on your specific regimen and how your skin is responding.

Stopping early may reduce effectiveness, so it is important to communicate with your care team rather than discontinue without guidance.

Response rates vary, and some patients may require additional treatment after completing a topical course.
